Attention-Deficit/Hyperactivity Disorder (ADHD) is a clinical term used to describe a neurodevelopmental disorder observed in both pediatric and adult populations. It is defined by core impairments in behavioural inhibition (self-regulation), sustained attention, distractibility and the regulation of motor activity in response to contextual demands, often resulting in observable hyperactivity or restlessness.

Historically, this condition has been referred to by a range of diagnostic labels, including hyperactive child syndrome, hyperkinetic reaction of childhood, minimal brain dysfunction, and attention-deficit disorder (with or without hyperactivity), reflecting the evolving understanding of its clinical features.

According to the DSM-5, ADHD is categorised into three primary presentations based on the predominant symptomatology:

1.     Predominantly Inattentive Presentation:

A marked difficulty in sustaining focus, particularly on tasks that require prolonged mental effort. This often manifests as distractibility, forgetfulness in daily routines and challenges with organization or task completion.

2.     Predominantly Hyperactive/Impulsive Presentation:

A pervasive sense of inner restlessness, frequently expressed through physical movement such as fidgeting, pacing, or an inability to remain seated. Speech may be rapid or excessive and the individual may appear constantly "on the go."

A tendency to act or speak without forethought, resulting in interruptions, difficulty waiting one's turn, or engaging in hasty decisions without considering consequences, may also be involved.

3.     Combined Presentation

This features both inattentive and hyperactive/impulsive traits.

Beyond these core features, many individuals experience:

  • Emotional intensity and dysregulation, including quick shifts in mood or heightened reactivity

  • Time management difficulties, sometimes described as "time blindness"

  • Episodes of hyperfocus, where intense concentration on preferred tasks contrasts sharply with distractibility in others

  • Sleep disturbances and heightened sensitivity to environmental stimuli

  • Working memory challenges, making it harder to retain and manipulate information in the moment
